Shanghai East International Medical Center offers pre-registration free of charge to the international community. Doing this in advance saves time and makes seeing the doctor more convenient should you become unwell. Simply fill out our registration form in person, or download and complete the appropriate PDF version below, and provide us a photocopy of your passport.

If you choose the PDF form, please remember to type in your information completely, then print out a paper copy and sign it. You may bring your completed registration form to us at any time, along with a photocopy of your passport. Please note that individual pre-registration may not be done by postal service, fax, or e-mail.
